Standard International protocol when patient presents between 0- 2 years of Age
- 0-3 months – pre surgical orthodontics
- 3 months – cleft lip nose repair
- 1 year cleft palate repair
- 5 years ? Pharyngoplasty/redo palate for speech if needed
- 7-11 years secondary alvelour bone grafting
- 13 years orthodontics
- 17 years orthognathic surgery
- 17 years rhinoplasty
- Distraction osteogonisis at any age
Protocols for repair of cleft deformities presenting between 2 – 7 years of age is as follows:
- 2 to 7 years: palatoplasty first and then lip repair followed by standard international protocol
- 7 years to adulthood: palatoplasty first and then lip repair with simultaneously alveolar bone grafting followed by standard international protocol
- When a bilateral cleft patient presents for surgery after the age of 7 years, the premaxilla may be severely displaced and may require surgical repositioning.
